How do wormholes work?

So, from what I understand wormholes are when two stars try to become Black Holes at the same time(or whatever plot device scifi writers feel is convenient) but how is the simple thing of two black holes being formed supposed to make a wormhole? I don't get what's supposed to suddenly connect them together.
